You will work across the Borders area within the Intermediate Care units within the Short term assessment and reablement team ( START). This post will provide assessments of need, provision of equipment, and treatment plans. The objectives of this role are to prevent unnecessary admission to hospital or support discharges from the BGH and Community hospitals. You will work closely with different professionals across both Social Care and Health and NHS Borders.The post holder will need to be flexible in their approach to developments in line with changes aross these organisations. A degree or equivalent in Physiotherapy and registration with the Health Professional Council is an essential requirement of the post. For informal enquiries please contact Shona Cavers, Team Leader.
