Job Purpose
>> Social Care Officers are key members of the team in the provision of person centred care and emotional and social support of individual residents during the night. The post holder is expected to take a lead role as a key worker in the development, recording, implementation and review of personal care and support plans in partnership with named individuals as directed by the Management Team. Social Care Officers provide support for individuals and their carers/families through active listening as well as demonstrating a genuine understanding of the rights and needs of older people
Main Tasks
- As key worker for named individuals, take responsibility for maintaining and updating of personal care plans, using both verbal and written recording reporting systems
- Assisting and/or enabling individuals to carry out; intimate personal care tasks, activities of daily living, as well as providing support in meeting social, spiritual and emotional needs
- To carry out and other general housekeeping duties as requested by the Management Team
