An offer of employment by Aberdeenshire Council will be subject to the outcome of this check being satisfactory. The preferred candidate will also be required to provide an Overseas Criminal Records Check from other countries as deemed necessary. An offer of employment by Aberdeenshire will be subject to the outcome of this check being satisfactory.

Job Purpose
>> To carry out the functions of assessment and care management to meet the needs of community care user groups in accordance with the requirements of the National Health Service and Community Care Act 1991.
Main Tasks
- To assess individual needs
- To prepare care plans to meet assessed needs
- To secure resources or services required to implement care plans
- To monitor the provision of care
- To review care plans to ensure needs are met
- To undertake financial assessments and budget-related activities
