The Hospice At Home Team encompasses the Access Team, Community Palliative Care Team and Virtual Ward Team. These teams provide specialist palliative care to people living with progressive life-limiting illness across Edinburgh and the Lothians.
We are looking for a Band 5 Staff Nurse to join the Virtual Ward Team. The Band 5 Nurse is a key member of the specialist multidisciplinary palliative care team, delivering high-quality, compassionate and evidence-based care to patients and those important to them.
The post holder will provide holistic assessment, planning, implementation and evaluation of palliative nursing care, addressing patients' physical, psychological, emotional, social and spiritual needs.
This position will be based within the Virtual Ward Team but, but depending upon service allocation and rotational requirements, the post holder may be asked to work within the Access and Community Palliative Care Team.
