Falkirk Health and Social Care Partnership is seeking to recruit a highly motivated, experienced and enthusiastic Advanced Practitioner Social Worker to join our East Locality Assessment and Care Planning Team.

This is an exciting opportunity to play a key role in delivering high-quality adult social work services while supporting the continued development of excellent practice across the team. As an Advanced Practitioner, you will carry a complex caseload and undertake strengths-based, outcomes-focused assessments and reviews in partnership with adults, carers and partner agencies. You will demonstrate expertise in risk assessment and risk management, applying professional judgement to support positive outcomes while promoting choice, independence and safeguarding.

Alongside your case management responsibilities, you will provide professional leadership within the team, supporting practitioners to develop confidence, skills and knowledge. You will contribute to practice learning, quality improvement and service development, helping to ensure high standards of professional practice across Adult Social Work Services.

You will work closely with colleagues across Health and Social Care Partnership services and partner organisations to develop innovative approaches that improve access to information, support and services for Falkirk residents.
