Job Summary

Post temporary for 12 months.

We are seeking a motivated and compassionate Care at Home Manager to join our Reablement Care at Home team. Key Responsibilities Lead and manage a reablement service supporting people at home. Focus on promoting independence and recovery after illness or hospital stay. Oversee care staff, assessments, and care planning. Work alongside health professionals to reduce long-term care needs. Ensure safe, person-centred care and effective discharge planning. The role includes peripatetic responsibilities, providing operational support across Care at Home teams as required, including assisting with absence management processes, undertaking home care reviews, and carrying out management investigations. About You A strong communicator who works effectively within a fast paced team environment. Demonstrate a strong commitment to promoting independence, dignity, and delivering high quality, person centred care. Able to lead, motivate, and support a diverse staff team. Manage performance and address practice issues. Manage competing priorities in a busy service. Coordinate care packages, staff, and resources efficiently.
