This role is ideal for individuals who are passionate about supporting people to achieve their personal outcomes and who can work flexibly across community and discharge pathways as part of a dynamic, multidisciplinary team.

The Community Care Assistant (CCA) within the Home Assessment and Reablement Team (HART) supports the delivery of high-quality, person-centred care and reablement services to individuals in the community, with a focus on maximising independence and achieving positive outcomes.

Working as part of a multidisciplinary team, you will contribute to timely assessment, care planning and review processes, supporting individuals to remain safely at home or return home following hospital discharge. This includes working within Discharge to Assess and prevention models to promote recovery, reduce reliance on long-term care, and support effective transitions across services.

This role will support rural areas within the North locality, with a hybrid base between Jessie Street, Blairgowrie and Pullar House.

You will have an active role in gathering and recording accurate information, monitoring progress, and identifying changes in need, ensuring that care and support is responsive and outcomes-focused. The role involves close collaboration with Coordinators, Reablement Assistants, Quality Monitoring Officers, and wider health and social care professionals to ensure continuity and coordination of care.

Through direct engagement with service users, families and carers, you will promote dignity, respect and choice, while supporting individuals to develop or regain skills for daily living. The role contributes to safeguarding, risk awareness, and the delivery of safe, effective care in line with organisational procedures, SSSC Codes of Practice and Care Inspectorate standards.

This is a fixed term post for up to 12 months to provide maternity cover.

Skills & Experience Required

Essential Experience

  • Experience of working within a health or social care setting, supporting adults with a range of needs including frailty, physical disability, or cognitive impairment.
  • Experience of delivering or contributing to person-centred care, including personal care and support with daily living activities.
  • Experience of working collaboratively with multidisciplinary teams (e.g. Social Work, NHS, Care at Home providers) to support coordinated care.
  • Experience of recording information accurately and maintaining clear, up-to-date records using electronic systems (e.g. Mosaic or equivalent).
  • Experience of identifying changes in need and escalating concerns, including supporting safeguarding and risk management.

Essential Skills

  • Strong commitment to person-centred, outcome-focused care, promoting independence and reablement.
  • Ability to communicate effectively with individuals, families, carers and professionals, building positive working relationships.
  • Ability to gather, interpret and share information to inform assessment, care planning and review processes.

Knowledge and Understanding

  • Understanding of reablement approaches, supporting individuals to regain or maintain independence.
  • Knowledge of self-directed support legislation and how this is delivered in practice.
  • Awareness of relevant legislation, guidance and standards, including SSSC Codes of Practice and adult protection principles.
  • Understanding of the importance of accurate documentation, confidentiality and information sharing.
  • Knowledge of local resources within communities would be an advantage.
  • Good organisational skills, with the ability to prioritise workload and respond to changing demands within a fast-paced environment.
  • Ability to work flexibly and independently, while contributing effectively as part of a wider team.
  • Awareness of risk, safeguarding and duty of care responsibilities, with the ability to act appropriately.

 Qualifications and Requirements

  • SVQ Social Services and Healthcare at SCQF Level 6 (or equivalent), or willingness to achieve within an agreed timescale.
  • You must have a valid UK driving licence, as this post requires travel to a variety of locations across Perth and Kinross.
