Full Job Description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included. 

Shift Pattern: 22.5hrs worked 9am-5pm over 3 days.

(Please note the salary is Pro Rata for part time hours.)

West Dunbartonshire Community Mental Health Services based in Riverview Resource Centre and Helensburgh CMHT are looking to recruit a Band 6 CPN for our small nursing service to improve Physical Health Care outcomes for patients with complex mental health needs in line with the NHSGGC Physical Health Care Policy.

The post holder will be integral to continue this service across the two Community Mental Health Teams in West Dunbartonshire, adhering to protocols and systems in place to ensure requirements of the Physical Health Care Policy are being met locally. The post holder will be expected to work in partnership with staff from local Community Mental Health Teams, GPs, practice nurses and third sector partners in delivering an effective, person centred and strength based approach to care. The post holder will be required run local Physical Health Care Clinics and undertake duties without direct supervision as well as supporting and supervising junior nursing colleagues. The post holder will be responsible for proactively promoting partnership working with patients and their carers. This will involve working in a variety of settings across the West Dunbartonshire catchment area and in some cases may include working within a patients home setting
