Mental Health Practitioner

We are looking for a compassionate and experienced Mental Health Practitioner to support our Primary Care teams across our two Aberdeen practices; Great Western Medical Practice and Whinhill Medical Practice. This is an exciting opportunity to work as part of a multidisciplinary team, providing timely mental health assessment, risk management and brief interventions for patients presenting with a wide range of mental health needs. You'll work closely with GPs and other healthcare professionals to improve access to mental health support, promote early intervention and ensure patients receive the right care, at the right time. What you'll be doing

• Undertake comprehensive mental health and risk assessments.
• Develop person-centred care and safety plans.
• Deliver brief interventions and ongoing support where appropriate.
• Provide specialist advice to GPs and the wider multidisciplinary team.
• Support patients with common and complex mental health presentations.
• Refer and signpost patients to community, voluntary and secondary care mental health services.
• Liaise with local mental health providers to ensure seamless patient care.
• Maintain accurate clinical records and contribute to service improvement initiatives.
