Due to the expansion of North Ayrshire Health & Social Care Partnership services linked to Crosshouse Hospital, we are seeking a motivated and compassionate Occupational Therapist to join our newly developed Discharge to be Assessed (D2A) model of care, which will include weekend working. This role supports the timely and safe discharge of people from hospital by enabling them to return to their home environment, where they can be assessed and supported in a meaningful, real life context. It is an exciting opportunity for an Occupational Therapist who enjoys community-based practice, with the majority of assessments and interventions carried out in people’s own homes and community settings, promoting independence and helping to prevent avoidable readmissions. As part of a supportive multidisciplinary team, you will carry out timely, strengths-based assessments, provide practical interventions, and make recommendations that enable people to live as independently as possible in the right place at the right time.
Key Responsibilities
- Undertake functional and environmental assessments for people referred through the D2A pathway, primarily within community settings.
- Work collaboratively with individuals, families, carers, and health and social care colleagues to support safe, person-centred discharges home.
- Deliver interventions that promote independence, safety, and wellbeing in everyday environments.
- Contribute to effective discharge planning and onward community support, helping to maintain patient flow while achieving positive outcomes.
- Record assessments, decisions, and care plans in line with professional and organisational standards.
About You
- HCPC-registered Occupational Therapist with evidence of continuing professional development.
- Confident in functional and environmental assessment, equipment provision, and community-based discharge planning.
- A strong communicator who works well within a fast-paced multidisciplinary team.
- Committed to promoting independence, dignity, and high-quality, person-centred care.
