Full Job Description

An excellent opportunity has arisen to join the Relief Care Assistant Team within Adult Services Resources, HSCP Integrated Complex and Critical Services. This service supports adults, some with profound and complex needs, mental health issues and autistic spectrum disorders as well as adults who have offending histories, both to live as independently as possible within their own homes and to help with inclusive integration and participation in their local communities.

You will support Service Users living at home with all aspects of their housing support and chosen lifestyle. This will include carrying out personal care, supporting service users at mealtimes, support with medication, moving and handling and be responsible for record keeping and finances. (This is not an exhaustive list).

This role is focused on individuals with learning disabilities and challenging behaviour, providing person-centred care, assisting with daily living activities and promoting independence, while managing behaviours that challenge, through positive and proactive strategies. You will implement and monitor positive behaviour support plans, using person-centred approach to manage behaviours that challenge. The role may include working within a multi-disciplinary team and may require specific skills in behaviour management and understanding learning disabilities, including but not limited to; CALM core theory, Escape, CALM physical intervention and CPI training.

You will adopt a person-centred approach in all aspects of service delivery and accompany Service Users in a wide range of personal interest activities and appointments while fully respecting their preferred lifestyle choices and while promoting their independence.

As a casual worker, you will be required to cover for permanent staff.

Lone Working may be a requirement for this post, which will mean you will work independently, taking responsibility for your shift and making decisions where required to ensure smooth and efficient service delivery.

The shifts that you will be required to cover will include dayshifts, backshifts, weekends, sleepovers and/or nightshifts.

Regular availability and responding at short notice is an expectation of this role, to continually meet the needs of the Service Users we support.
