Full Job Description
To provide and apply a regulated Care service in line with Health & Social Care Standards.
To assume case situation responsibilities specific to the service, delivering within live timeframes and assessments of situations using risk assessment tools.
To follow service procedures, delivering direct interventions to staff in relation to client's specific demands and needs, focusing on obtaining positive outcomes and offering of solutions within a structured approach using our systems.
To maintain, audit accurate records of service delivery and report all information according to the policies and procedures within the framework of the business.
To Mentor, support and empower new staff in locality to achieve confidence through development, observation, guidance and by being a proficient and approachable Senior role model.
RESPONSIBILITIES
Deliver service outcomes in an efficient and effective way, ensuring compliance to meet business and legislative requirements.
To support inexperienced staff to develop their skills and knowledge enabling them to be proficient in delivering personal care and person-centered activities in a manner which is in line with Nightingale Home Cares values.
To coordinate and schedule client and staff delivery in the absence of the Care Coordinator/scheduler, with the use of the integrated systems.
To collaborate in the development of client care plans.
To represent the business in a professional manner within client's homes
JUDGEMENTS AND DECISIONS
To liaise with the management team in relation to any acute staff issue.
Contribute to the construction, implementation, monitoring, and review of appropriate care plans whilst collaborating with colleagues and other agencies to achieve the best outcomes for the clients.
To participate in staff induction, training, and ongoing skills development of all Home Care Staff. Contribute to the sourcing and accepting of new business.
Professional qualification in Health & Social Care, SVQ 2 essential, with a desire to complete SVQ3.
Registration with the appropriate professional body
Use of software systems to coordinate care rosters and schedules.
Computer literate with an ability to record appropriately and produce suitable documents
Good organisational skills and the ability to prioritise demands
Ability to work with other agencies and disciplines in a partnership and collaborative approach
To provide a valuable senior support to the carers in the field and out of hours
Ability to apply confidentially and share information appropriately
Good understanding of situations which affect clients using the service.
Ability to behave in a consistent and reliable manner and be a provider of practical support and learning to the Home Care Assistants.
Ability to travel efficiently and effectively where required.
Ability to provide personal care when required.
To manage own workload to maintain momentum and achieve performance management.
To participate in on call duties out of hours.
