What is the job function?
An exciting opportunity has arisen for a nurse to join our Health and Wellbeing Team.
You will carry out health assessments and support high-quality care for young people with a range of needs, including complex health conditions, risk-taking behaviours, neurodevelopmental disorders, and mental health difficulties. Using your clinical judgement and public health knowledge, you will identify health needs, contribute to health action plans. Working with residential staff to support the delivery and review of the identified health actions.

Responsibilities
- Undertake comprehensive health assessments for young people, assessing physical health, emotional wellbeing, medication, developmental needs and lifestyle factors. Produce detailed reports and action plans to inform care planning
- Assess young people presenting with health concerns and provide practical guidance to residential staff on how to support identified health needs
- Facilitate access to outreach NHS services within the residential setting (e.g. sexual health, immunisations, primary care and specialist services) and support young people to engage with these services.
- Attend and contribute to multi-disciplinary meetings, case formulations, and mental health assessments as required, advocating for the health needs of young people and contributing to care planning.
- Contribute to Neurodevelopmental Disorder assessments and prepare written reports in collaboration with clinicians from CAMHS and FCAMHS where appropriate.
- Promote healthy lifestyles and wellbeing, in line with national health promotion and mental health strategies, providing advice and guidance to young people, residential staff and education teams.
- Demonstrate knowledge of the determinants of health and ill health, providing guidance and leading on initiatives to support health education, prevention and health improvement across the services.
- Provide support during critical incidents, including responding to self-harm or suicidal behaviours, and work alongside Specialist Intervention Services to support risk management and care planning.
- Contribute to and oversee the safe management of medication, including maintenance and monitoring of the Electronic Medication Management System.
- Develop and deliver training to residential staff on health-related topics such as safe handling of medication, responding to self-harm and recognising health concerns.
- Provide cover for the Lead Nurse when required.
Communication and Relationships
- Develop and maintain therapeutic relationships with young people in the residential setting.
- Communicate information regarding the management of mental and physical health needs to colleagues, multidisciplinary professionals, young people and carers in a clear and appropriate manner.
- Communicate with young people and their relatives using a range of communication approaches appropriate to their age, understanding and needs.
- Adapt communication when working with young people who have impaired communication due to developmental disorders or mental health needs
- Maintain professional and effective communication with colleagues within the residential team and wider multidisciplinary services.
- Communicate appropriately with colleagues from partner organisations involved in the young person’s care.
- Practice and promote confidentiality in line with the NMC Code.
- Provide support to young people who may be distressed, upset, angry or anxious.
- Use communication and de-escalation skills to manage conflict situations, ensuring the safety of self and others.
- Ensure written documentation and records are clear, accurate and maintained in line with the NMC Code.
Qualifications, Skills and Experience
Essential: Registered Nurse with current Nursing and Midwifery Council (NMC) registration.
- Desirable: Registration in Children’s Nursing (RN Child) or second registration in Mental Health Nursing (RMN).
Experience
- Essential: 3 years Post-registration experience in a relevant clinical setting (e.g. school nursing, primary care, paediatric services or paediatric A&E).
- Essential: Experience working with children or young people with complex health, developmental or emotional needs.
- Desirable: Experience working with vulnerable children or those in residential care
Skills
- Confident in recognising and responding to the physical and mental health needs of young people, including those in secure care settings.
- Demonstrates meticulous attention to detail, particularly in relation to medication management.
- Demonstrates a sound understanding of safeguarding principles for children and young people.
- Ability to undertake complex clinical assessments and develop care plans, particularly for young people in secure care settings.
