Full Job Description

We are looking for a Support Practitioner to join us on a part Time basis (25 hours) at our TouchBase Fife Adult Day Service based in Kirkcaldy.

Our service is open 6 days a week (0800 - 1700 hrs) and we are looking for staff to work flexibly on our rota to meet the needs of the People we support.

We are passionate about developing the people we support's outcomes and achievements in life. The individual(s) you will support may have limited communication, therefore we would require you to work hard to develop and understand how each person communicates.

Your role will be centred around building trusting relationships as a foundation to assist with practical tasks such as :

Support and monitor people supported with their healthcare needs, including administering medication and personal care

Promote independence and individuality according to individuals Support Plan

Record daily tasks and maintaining accurate up to date records

Work as part of a team, effectively communicating with staff team, families and people supported

Each individual has their own interests and hobbies, successful candidates will be supporting individuals in their chosen activities and helping them to develop new activities and relationships where possible.

Each individual has their own activity planner which can include activities in TouchBase or accessing the community.
