Full Job Description

An exciting opportunity has arisen within our Dunfermline and West Fife Hospital at Home service for a highly motivated and experienced Advanced Nurse Practitioner to join the team.   This role will support the H@H team to deliver safe, effective and person centred care to patients within the primary care setting. The role will predominantly focus on the Urgent and Unscheduled Care needs of the older adult population and the complex treatment and management of acutely unwell, palliative or frail patients including: nursing and care home residents.

It is recognised that as people live longer, services and the way we work together has to change if we are to continue to meet the needs and aspirations of our communities.  In Fife Health and Social Care Partnership there is a recognition that having staff with the right knowledge and skills within the community is required to support patients to live long and healthy lives in their own home or in a homely environment. 

Advanced Nurse Practitioner candidates must hold a Postgraduate Diploma in Advanced Practice at Masters level and have been assessed as competent in this level of practice.

If you do not currently hold a Diploma in Advanced practice you will be required to be educated to Post Graduate Diploma (Level 11) in Advanced Practice, including Advanced clinical assessment and decision making modules and have completed a Post Graduate Diploma in Advanced Practice (Level 11) Non-Medical prescribing. You would then be fully supported with additional training in your advanced practice diploma with a planned completion date of 12-18months.  This is dependent on the successful candidates previous academic study as there may be an opportunity to RPL previous academic credits towards the PG Dip in Advanced Practice.  This will be done under Annex 21.

Additional training in leadership/management would be beneficial but not essential.  

You should have excellent communication skills and be able to work as a member of the wider Health and Social Care Team with the ability to work autonomously utilising your own initiative, clinical judgement and decision making.  As this is a community post you will also be required to travel across Fife.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  This advert and any subsequent offer/contract of employment therefore reflects the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns, please contact the Recruiting Board.
