Full Job Description

We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.  

The shift pattern is 5 days over 7.

An exciting opportunity has arisen for a Registered Nurse who has an interest in Rehabilitation to work within an interdisciplinary Community Rehabilitation Team providing rehabilitation for patients in their own home.

This post involves working as an autonomous practitioner within a multi-disciplinary team, and independently, carrying out comprehensive holistic MDT and nursing assessments. You will be responsible for planning treatment and care and supporting hospital discharges and admission avoidance referrals.

The post-holder should have excellent communication skills and be able to liaise with patient, carers, team members and other agencies to co-ordinate and support the patient throughout the service.

Experience in Adult Rehabilitation, Neurology, Orthopaedics, Medicine or Care of the Elderly Specialities is desired. 

Car driver is essential and Community Experience would be beneficial.
