Homecare & Reablement Worker, Barvas to Ness, Western Isles (5087)

16 hours Per Week, Permanent post

£12,523 - £13,741 per annum, inclusive of £1,185 per annum Distant Islands Allowance

Western Isles Council - Home Care and Reablement Service is looking for care staff who are caring, compassionate and reliable to join our team of skilled staff to support people living in our communities and enabling them to continue living at home. The service aim is to ensure a very good or excellent standard of care and support is provided at all times, based on person centred planning, compassion, dignity, respect and trust. The Home Care and Reablement Worker will undertake a key role in the development of care plans which meet the needs of service users. The post holder will contribute to the risk assessments and maintain and monitor the risk management plans and medication administration plans as appropriate. The Home Care and Reablement worker will utilise these sources of information in their role within Home Care and Reablement; ensuring that the personal plan is developed, maintained, understood and available for the Care and Support supervisor to act upon, at the time-of-service user review.

The post holder will also provide direct care and support to service users living within their own home (or within a homely setting). This will involve the provision of a range of care and support tasks as detailed in the care and support plan to maximise service users’ independence, supporting Reablement and Intermediate Care Provision and thereby supporting Service Users to remain in their homes (or homely settings). The range of care and support tasks and activities will be undertaken as detailed in the care and support plan and in accordance with the relevant risk assessments and management plans. The workload will be agreed by the Care and Support Supervisor.

This post requires a qualification at SVQ 2 in Health and Social Care or equivalent qualification to enable registration with SSSC (Scottish Social Services Council).

A full UK Driving License is required.

The wearing and use of PPE is essential in this post.

The salary above is inclusive of Distant Islands Allowance (pro rata).
