This is an exciting time to join the Falkirk Health and Social Care Partnership and we are now looking to appoint a Homefirst practitioner to our Hospital discharge team.  You will be a motivated worker who is both enthusiastic and committed to improving the lives of our service users, who practices in a way that promotes better outcomes and focuses on strengths, rather than deficits. You will be primarily based at the acute hospital site at Forth Valley Royal. This is full time hours, 8am to 4pm, and permanent. Homefirst is a 7 day service with workers working 5 days over 7, which includes a rota for weekend work.

Homefirst practitioners work in a high paced reactive and responsive environment as part of the Health and Social Care Partnership. This role includes partnership working to review and support discharge planning for service users. The team facilitate timely discharges to home, or to intermediate care, working in partnership with the integrated health team and to achieve the best outcome for service users

You will join our multi-skilled team; working closely with colleagues from within the health and social care partnership, in addition to a wide range of external partners.
