Full Job Description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.  

The shift pattern is Monday-Friday 09.00-17.00 (37.5 hours).

The Post Diagnostic Support (PDS) Link Worker is a post open to both health and social work staff. It will initially be based in the Older Peoples Community Mental Health (OPCMH) Service at the Royal Alexander Hospital (RAH), part of the Adult Mental Health Services, within Renfrewshire HSCP. The PDS Link Worker will deliver a minimum of one years Post Diagnostic Support to those people newly diagnosed with dementia. The PDS Link Worker will enhance the support already available from the Renfrewshire Older Peoples Community Mental Health Team. Link Workers provide Post Diagnostic Support in line with the 5 Pillar Model for people who have received a diagnosis of dementia and will work with families for a minimum of a year, providing advice and support around aspects which will enable people to live well with dementia for as long as possible.
